Jun 14, 2021 at 1:46 comment added BobT Because the bleach step is supposed to completely remove the negative silver image and leave the unexposed halide alone. Your processing didn't remove most of the negative silver image. The re-exposure step worked- the film is black in the margins, but without removing all of the initial development's silver you had both a negative image and a positive image superimposed on each other.

Jun 13, 2021 at 23:21 comment added BobT I think it most likely WAS something up to and including the bleaching stage that caused the problem. Underexposure and/or 1st development problem and/or bleach problem.
