comment What is the formula for percent of frame filled at a specific distance and focal length?
Sure, but focal length is quite universally in millimeters, so it makes sense to measure sensor size in millimeters as well.

Actually you could have distance and subject_size in larger units, as long as you use the same unit for both.

comment What kind of lighting equipment should I have as a Strobist beginner?
What about gels? I'd say at least CTO gel has been very helpful in my flash photography; without gels, flash usage would be limited to daylight or total darkness.

comment Which DSLR camera can I go for?
If you are really going into professional photography doing macro, I'd suggest you consider a Canon system where you'll be able to use the macro photo lens with largest magnification (5:1), the MP-E 65.

comment How can I maximize the “blurry background, sharp subject” (bokeh) effect?
The lens does have f/2.8 aperture, but its APD elements cause light loss, therefore your exposure will be similar to f/4.5. APD elements enhance smoothness of bokeh, but won't let you get away with using a smaller aperture.

Infrared-based proprietary wireless systems also have the downside of requiring line of sight between camera and flash.

comment What photographer took this mid 20th-century color photograph?
@mattdm haven't voted myself, but I can see that the question is on borderline of image recognition, which is generally considered off topic (even by yourself) - see meta.photo.stackexchange.com/questions/1283/…
